Skilled Nursing

Our skilled nursing services provide professional medical care in the comfort of your home, tailored to meet individual health needs and ensure well-being.

  • Provide and document skilled nursing care, including medication administration, wound care, and patient education.
  • Educate patients and families on health management, safety precautions, and self-care techniques.
  • Nursing services, treatments, and preventative procedures, requiring substantial specialized skill, and ordered, by the physician
  • The initiation of preventative and rehabilitative nursing procedures, as appropriate, for the patient's care and safety.
  • Observing signs, and symptoms, and reporting to the physician: reactions to treatments, including drugs, as well as changes in the patient's physical, or emotional, condition.
  • Teaching, supervising, and counseling the patient, and caregivers, regarding the nursing care needs and other related problems of the patient, at home.
  • Coordinate patient services.
  • Provide guidance and supervision to the LPN and supervises the LPN once monthly.

 

 

 

 

 

 

 

 

 

Home Health Aide

Our dedicated home health aides offer compassionate assistance with daily living activities, ensuring comfort, safety, and independence for our clients.

 

  • Follow the plan of care to provide, safe, competent care to the client.
  • Help the patient maintain good personal hygiene and assists in maintaining a healthful, safe environment.
  • Plan and prepare nutritious meals, markets, when instructed to do so by the nurse.
  • Assist the patient with ambulation, as ordered by the physician, and approved and supervised by the nurse.
  • Assist the therapy personnel, as needed, with rehabilitative processes.
  • Encourage the patient to become as independent as possible, according to the nursing care plan.
  • Give simple emotional, and psychological, support to the patient, and other members of the household, and establish a relationship with patient and family, which transmit trust and confidentiality.
  • Report any change in the patient's mental, or physical, condition, or in the home situation, to the staff nurse, or to the Aide supervisor.
  • Prepare a visit report, promptly, and incorporate same in the clinical record, weekly.

 

Therapy Services

We offer physical, occupational, speech, and respiratory therapies to help our clients regain independence, improve function, and enhance their quality of life.

  • Assist the physician in evaluating patients, by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
  • Consult, and familiarize, staff regarding the established plan.
  • Teach, supervise, and counsel the family, and patient, in the physical therapy program.
  • Instruct patient in care and use of wheelchairs, braces, crutches, canes and prosthetic, and orthotic devices.
  • Provide the equipment required to carry out the plan.
  • Arrange for out-patient service, for procedures that cannot be given at home.
  • Prepare an assessment of the patient's condition and periodic evaluations and summaries.
  • Supervise Physical Therapy Assistants and home health aides, as per Agency policy.
  • Record, and report to the physician, the patient's reaction to treatment and /or any changes in the patient's condition.
  • Communicate with the physician, at least every 30 days, or whenever changes occur, regarding the patient’s status; obtain additional orders, if necessary.
  • Communicate with other team members (including Home Health Aides) and, when appropriate, instruct them in certain phases of physical therapy they may use, while working with the patient.
  • Write clinical/progress notes, on the date of the visit, and incorporate same, in the patient's record, within 48 hours.
  • Participate in case conferences, physical therapy staff meetings, Agency activities, and in-service programs, as necessary.
  • Supervise the Home Health Aide, every two weeks, when physical therapy is the only professional discipline involved in the patient's care and supervise the Physical Therapy Assistant, at least monthly.
  • Participate with staff, patients, and physicians in discharge planning activities and complete a Physical Therapy Discharge Summary.

Attendant Care & Homemaker Services

Our attendant care and homemaker services provide essential support with personal care and household tasks, ensuring a comfortable and well-maintained living environment.

 

Follows the service plan to assist the client in maintaining a healthful, safe environment.

Performs ONLY those functions specified for each individual client on the service plan.

Receives written instructions from the Agency Manager or designee.

Has knowledge of Agency policies and procedures.

Is oriented and trained in all aspects of services to be provided to clients.

If able to demonstrate competency in all areas of training for client services.

Assists clients in the home to carry out functions of daily living, self-care and mobility.

Will assist clients with the following activities within the personal service attendant scope of services:

a.    Assistance in getting in and out of beds, wheelchairs, and motor vehicles.

b.    Assistance with routine bodily functions, including:

1.    Bathing and personal hygiene

2.    Using the toilet

3.    Dressing and grooming

4.    Feeding, including preparation and cleanup.

5.    The provision of Medication Assistance:

i.              Through providing reminders or cues to take medication.

ii.            The opening of preset medication containers.

iii.           Provide assistance in the handling or ingesting of medications, including controlled substances, prescription drugs, eye drops, herbs, supplements, and over-the-counter medications.

c.    Arranging for transportation to and from appointments

d.    Administering emergency first aid

e.    Providing or arranging for social interaction

f.     Providing transportation

g.    Shopping

h.    Assisting with paying bills

Reports any change in the client's mental or physical condition or in the home situation to his/her immediate supervisor or Agency Manager.

Completed client paperwork on the day service was provided and submits timely, per agency policy.
